Save the date! Join us on September 15th to celebrate CHCACT’s Practice Transformation Network(PTN) initiative at the “Connecticut Keys to Practice Transformation Network (PTN) Summit” held at the Chrysalis Center. If you would like to attend, kindly RSVP here by September 1, 2017.
This year the Summit will focus on a culture change in the way Federally Qualified Health Centers (FQHCs) provide care to patients, to accommodate the shift towards alternative payment models. Content will center around the idea of creating an environment that supports and sustains a culture of quality, from leadership down. Health Centers will also provide reports on their commitments to Diabetes Care and team based care management.
Honorable speakers include Centers for Medicare and Medicaid Services(CMS) Transforming Clinical Practices initiative(TCPi) Medical Director Dr. Paul Rosen, as well as faculty presenters Dr. Karen Funk of Colorado and Dr. Kirsten Meisinger of Massachusetts. Both CMS faculty presenters are from FQHC environments, which makes them especially relevant to the audience in the Connecticut PTN. They will speak to the successes and challenges they have faced in implementing similar models of care.
